I/We hereby apply to Grimsby Power Incorporated for electric service(s) at the service address shown above. I/We agree to pay for such service(s) as bills are rendered and comply with the conditions for service of Grimsby Power Incorporated.

I/We agree to: pay the security deposit required; or provide a 5 year electric utility credit history for service under 50kW or a 7 year electric utility credit history for service over 50Kw to waive the security deposit. I/We understand that if my/our payment history is not acceptable, I/We am/are required to pay a security deposit. I/We further understand that in the event of either non-payment of deposit or account, service may be terminated and not restored until payment is made in full including charges for reconnection. I/We will also inform Grimsby Power Incorporated at least one week in advance of when I/We plan to vacate the above premises.

I/We have read and understood that if the property no longer has active services, Grimsby Power Incorporated may contact the owner of the property (if different from the account holder) to notify them of the status for the safety and security of the property.

Grimsby Power Incorporated will make every effort to ensure bills are accurate; however, billing errors can occur. Grimsby Power Incorporated reserves the right to collect under-billed amounts in accordance with section 7.7.7 of the Retail Settlement Code (RSC).

I/We have read and understood the attached statement of Grimsby Power Incorporated regarding the collection, use and disclosure of my/our personal information, and hereby consent to have Grimsby Power Incorporated collect and use personal information for the purposes stated.
